A community nursing team covering Paignton and Brixham has scooped an Outstanding Care Award.
Paignton and Brixham Community Nurses, employed by Torbay and South Devon NHS Foundation Trust, were nominated by Grange-Lea Care Home in Paignton for the recent Outstanding Care Awards of 2019.
The Outstanding Care Awards for Devon and Cornwall celebrate the most dedicated individuals, providers and suppliers in the care sector. The awards are backed by Proud to Care, Devon; Devon Country Council, Torbay and South Devon NHS Foundation Trust and Torbay Council.
The nurses have been recognised for all the support they give to Grange Lea throughout the year.
The team provides training and on-going support to help the home provide excellent care for patients under end-of-life care and how to manage the care of a patient who had complex needs in between their routine community nursing visits.
Jill Liardet, Torbay and South Devon NHS Foundation Trust’s Lead Community Nurse for Paignton and Brixham, said: “The nurses work in partnership with many homes across Paignton and Brixham to support end-of-life patients. The nurses are really pleased to be recognised for this award as they pride themselves on supporting care homes to deliver high quality care.”
Nicola Tucker-Stone, Registered Manager of Grange-Lea, described the nurses as being highly motivated, caring and passionate about their role delivering a truly person-centred service. They get to know residents and adapt their approach to suit and support individuals.
The nurses also ensure residents are involved in their own treatment plans and suggest to residents how to help themselves and encourage autonomy and independence.
Nicola said: “They are exceptional regarding end-of life-care, they offer a holistic approach, are always respectful of cultural and spiritual needs, give information to loved ones to keep them informed in an understandable and sensitive format. They also support the emotional wellbeing of families, friends and staff. Family members are frequently commenting how wonderful they are. The nurses go above and beyond to ensure people have a dignified, peaceful and pain-free death.”
She added that her care home staff felt confident in approaching the nurses for support and their expert advice. The nurses encourage staff to ask questions and are inventive in helping the home find ways to improve
