The focus of Jack Sears is on supporting an active recovery for all individuals with recent changes to their health needs to support with regaining independence and returning home swiftly.

The rehab support is provided by Torbay and South Devon NHS Foundation Trust and comprises of a therapy lead, an assistant practitioner, physiotherapist, occupational therapist and our discharge coordinator who are based at the unit.

They work alongside the Jack Sears care team and manager, and this maintains a consistent staff group and continuity for the people in being treated in Jack Sears which helps achieve the best outcomes possible. In addition, the community nursing team support Jack Sears, alongside dietician and pharmacy support and the local GP practices.

The team at Jack Sears work closely to develop a person-centred plan to aim to get patients home within 14 to 18 days, however if a person needs longer this is accommodated to achieve the best outcome for the person.

Shelly Machin, Care Group Director for Torbay and South Devon NHS Foundation Trust, who was part of the visit that took place on Thursday 30 January, said: “Jack Sears is here to help people avoid hospital admission or needing long term placement in a care home; this centre also helps us free up space within our emergency pathway.

“It really is an excellent rehabilitation centre. All the rooms are bright, spacious and modern, the food is excellent and the staff extremely attentive. Jack Sears is run with patient care as the priority.”

Anna Coles, a Divisional Director for Adult Social Care at Torbay Council who was part of the team visiting Jack Sears said: “Jack Sears is a fantastic example of partnership working between Torbay Council, the NHS Trust and Pendennis the independent care provider.”

“It provides a modern therapy led environment which helps our residents in Torbay to regain their independence following a stay in hospital or following a deterioration in their health at home. It has been wonderful today to meet some of the residents and the dedicated staff team who provide high quality rehabilitation and enabling support.”

Nikki Barnes, manager of Jack Sears said: “We are incredibly proud of Jack Sears House; our dedicated staff, the work we do, and the positive outcomes we achieve for our residents.

“Our collaboration with the NHS is truly inspiring and benefits everyone involved. We embrace a ‘doing with, not for’ approach, and we see its impact every day. We have worked hard to create an environment that is both welcoming and comfortable while also supporting residents in their journey toward independence”
